Additional extensions for test results, surgery scheduling or prescriptions are provided when calling. If you need to fax a document the number is: 425-391-6464.
The office is located in the N.E. Gilman Professional Center, conveniently located just minutes from I-90 with plenty of free parking.

We are taking new patients: Dr. Michalak and our advanced healthcare practitioners are available for new patients. Most general dermatology appointments start with evaluations by either a physician assistant or advanced nurse.
We offer Telemedicine: This service is easy to set up requiring a computer or smartphone. The appointment is treated like an office visit, and billable to insurance if applicable. Please visit our telemedicine page for more information.
Mask Policy: We are asking anyone entering the office to please wear a mask or face covering until further notice. We acknowledge that while most businesses are not requiring masks at this time, we feel as a healthcare facility it’s still the best way to keep patients safe. Cancellation/No Show Policy: Due to the extended wait times for scheduled appointments, we require 24-hour notification for cancellations so we may accommodate other patients trying to get in to see us. No shows will be assessed and are subject to a $50 cancellation fee.
